Requirements here apply identically to DOT and non-DOT populations — there is no separate DOT audiometric standard for the hearing conservation program itself. - Pure-tone air conduction testing at 500, 1000, 2000, 3000, 4000, and 6000 Hz in both ears, with 8000 Hz included as clinical best practice.
- Hughson-Westlake bracketing — down 10 dB after a response, up 5 dB after no response — with threshold recorded when the employee responds to at least two of three presentations at the lowest level.
- Baseline audiograms that anchor every future comparison for that worker, and annual audiograms compared against them.
- Standard Threshold Shift calculation, averaging change at 2000, 3000, and 4000 Hz against baseline.
- Otoscopic examination before testing for cerumen occlusion, foreign objects, or active drainage.
How we run it
The standards we hold, including where they exceed the rule.
- Ambient noise governs the mobile setting. Octave-band levels are verified against the 1910.95 Appendix D limits before baseline testing begins at any new site. A site that cannot meet the limits is not tested until it can.
- Baseline integrity is protected. A baseline collected in a non-compliant environment or without a genuine noise-free window is invalid, and we retake it rather than carry a bad anchor forward.
- Biological calibration runs every day the equipment is used. If any frequency deviates by 10 dB or more, testing stops immediately and the unit comes out of service. Full external calibration happens every 12 months.
- Escalation is not optional. Any Standard Threshold Shift or clinical abnormality routes to an audiologist or occupational physician. Technicians do not clear their own abnormal results.
- We verify the employee has been free of workplace noise for at least 14 hours before a baseline test. Where that window cannot be met for an annual test, we document hearing protection use during the preceding shift.
- On a confirmed persistent shift, we notify you in writing within 21 days, and deliver written results and counseling to the employee in the same window.

- Ambient noise verification with a Type 2 sound level meter before we commit to boothless audiometry at your site. This is a go/no-go gate, and we run it during site survey.
- A scheduling window that gives employees 14 hours away from workplace noise before baseline testing — often first thing on a Monday, or after a scheduled day off.
- A parking position for the trailer far enough from active noise sources to hold the Appendix D limits.
- An optional confirmatory retest within 30 days for any shift found, to rule out a temporary threshold shift before it is recorded as persistent.
Results live in the Occupational Health Record, separate from the personnel file. Supervisors receive fitness-for-duty and restriction information only — never underlying results or diagnoses. Employee medical records are retained for the duration of employment plus 30 years, per 29 CFR 1910.1020.
